Machine Learning for Automated Calculation of Vestibular Schwannoma Volumes.

Hypothesis Machine learning-based algorithms are capable of automated estimation of vestibular schwannoma tumor volumes without user input. Background: Volumetric tests are the most sensitive for the detection of vestibular schwannoma development, and they are particularly relevant for patient counseling and management decisions. Methods We developed a deep learning framework that used transformers and convolutional neural networks to determine vestibular schwannoma volumes without operator input. Retraining on the internal data set resulted in a Dice score of 82% when compared to manually segmented images, and 85 percent when only considering tumors of a similar size as the external data set. Conclusion Sophisticated machine learning algorithms delineate vestibular schwannomas with an accuracy that exceeds established standards of up to 20% error for repeating manual volumetric measurements 87% to 85 percent on a more varied dataset reflecting real world neurotology use.

Reduction of sporadic and neurofibromatosis type 2-associated vestibular schwannoma growth in vitro and in vivo after treatment with the c-Jun N-terminal kinase inhibitor AS602801.

Objective Vestibular schwannomas are benign nerve sheath tumors that result from a mutation in the tumor suppressor gene NF2, with functional impairment of the protein merlin. c-Jun N-terminal kinase is constitutively active in human VS cells and plays a vital role in their survival by preventing mitochondrial superoxide accumulation, thereby preventing the accumulation of mitochondrial superoxides, implicating JNK inhibitors as a potential systemic therapy for VS. The authors hypothesized that the adenosine 5'-triphosphate-comparative JNK inhibitor AS602801 would have antitumor activity in multiple VS models. In male athymic nude mice from newly collected human tumors, VS xenograft tumors were established. Tumor volume and cell proliferation decreased in mice treated with human VS xenografts, which reduced tumor number and cell proliferation in mice. Conclusions JNK inhibition with AS602801 has slowed the growth of sporadic and neurofibromatosis type 2-associated VSs, according to the findings.

Although GKRS has been shown to reduce tumor formation, the effect of GKRS on the symptoms caused by TS has not been sufficiently investigated. Since GKRS, 17% of the patients with diplopia had improved symptoms 12 months after GKRS, and 50% of the patients recovered after 24 months. Conclusions GKRS can be an effective treatment modality for TS tumor management, as well as facial pain, showing promising results in reducing TS-related symptoms, especially facial pain.

Intracapsular Enucleation of Cervical Sympathetic Chain Schwannoma.

Objectives Schwannoma arising from a cervical sympathetic chain is a rare clinical entity. CSC is impossible to maintain in schwannoma surgeries, resulting in Horner's syndrome and first bite syndrome. This essay is intended to discuss our efforts in preserving CSC in CSCS surgeries. METHODS This is a retrospective examination of CSCS cases treated at our Tertiary medical center from Apr 2018 to March 2022. CSC cryotherapy storage Only cases with excellent intraoperative preservation of CSC were included. There were 2 male patients and 1 female patient. Patients recovered from surgery for all three patients, but no FBS were present in any patient. When intracapsular enucleation was successful, postoperative FBS could be avoided, while postoperative HS could occur, but the recovery was satisfactory.

Quantitative Chemical Exchange Saturation Transfer Imaging of Amide Proton Transfer Differentiates between Cerebellopontine Angle Schwannoma and Meningioma: Preliminary Results.

We retrospectively reviewed nine patients with schwannoma and nine patients with meningioma who underwent APT-CEST MRI from November 2020 to April 2022 pre-operation.
