Pentamidine-Induced Derangements of Glucose Homeostasis: Determinant roles of renal failure and drug accumulation: A study of 128 patients

"RESEARCH METHODS" was a book that looked at blood glucose levels in 244 consecutive immunocompromized patients with Pneumocystis carini pneumonia: 116 patients were treated with cotrimoxazole, and 128 others were treated with pentamidine, with cotrimoxazole and 128 others receiving pentamidine. RESULTS Two cotrimoxazole patients developed diabetes as a result of necrotizing pancreatitis, while others remained euglycemic. Patients treated with pentamidine were hospitalized with severe glucose homeostasis disorder: hypoglyce-mia in 7, hypoglycemia and then diabetes in 18, and diabetes alone in 23 out of 58 patients. The B-cell secretory responses to stimuli were poor, and 26 of the 41 diabetic patients required insulin therapy; their plasma C-peptide levels were lower than average; and their B-cell secretory responses to stimuli were poor. Patients treated with pentamidine differed from their euglycemic counterparts by higher pentamidine doses, elevated plasma creatinine levels, and more severe anoxia and shock were all present. Pentamidine-induced dysglycemic incidents are chiefly due to inadequate insulin delivery and toxicity to the islet B-cells, according to CONCLUSIONS.
