"Mixed epithelial and stromal tumors of the kidney are a multi-layered neoplasm in which malignancy seldom arises. " We report four mixed epithelial and stromal tumors in which sarcoma or carcinoma developed in this research. Clusters of small benign glands surrounded by oestrogen receptor-positive benign stroma were present outside the renal parenchyma, supporting the diagnosis of mixed epithelial and stromal tumors. The presence of benign looking branch tubules in fibromuscular stroma, as well as smooth muscle actin, desmin, and progesterone receptor, arose in the diagnosis of mixed epithelial and stromal tumor in which a small round blue cell sarcoma with EWSR1 rearrangement was confirmed. In a typical mixed epithelial and stromal tumor, adenocarcinoma with papillary architecture arose in the fourth tumor, adenocarcinoma with papillary architecture. We review four cases of mixed epithelial and stromal tumour with malignant change, two of which are carcinomatous, and the other two have sarcomatous transformation.
