A Novel Healthcare Information Technology Tool to Improve Care in Patients

This report is a prospective, observational study that seeks to investigate the use of the Geneva Healthcare Patient Population Management platform to detect AF in patients with CIEDs in the emergency room environment, as well as the possibility of sub-specialty cardiac electrophysiology care of these patients. The investigators intend to use the Geneva Healthcare Patient Population Management platform to find AF in patients with CIEDs, and they will use a clinical excellence team of nurse practitioners and research assistants to ensure that appropriate care of AF is sought. Both emergency room sites at Hillcrest and Thornton have used the Geneva Patient Population Management Platform for medical use since September, 2012. The information obtained from the CIED will be uploaded by the hospital staff to UCSD's electronic medical record system, EPIC, after the data has been processed and organized on the password protected platform. Investigators want to screen all patients in any of the two UCSD emergency departments that have no cardiac rhythm monitoring devices. Patients with AF will have data collected by the Geneva Healthcare PPM platform from the point-of-care interrogation in the ER or home monitoring. The investigators suggest a pilot clinical excellence research using emerging healthcare technology technologies to identify patients with AF and CIEDs and efficiently route these patients for guideline-based therapy with a target of efficient radiofrequency catheter ablation of AF and/or medical therapy.

Patient and Healthcare Professional Views on Genetic/Genomic Information and Testing

Both quantitative and qualitative data from healthcare professionals as well as patients will be collected and analysed in the study. Patient education resources will be developed based on healthcare professionals' experience and patients. Using the gold standard Patient Education Materials Assessment Tool, quality of materials will be quantitatively measured. Utilizing an expert clinician's report, identify the key components of a patient decision aid. Healthcare specialists who are content experts in the field will be interviewed to gather key data for a new genetic testing decision aid. Quantitative findings will influence the design of a patient decision aid for genetic testing. Patients' qualitative focus groups will be administered and analyzed using thematic analysis. Findings will aid in the establishment of a patient decision aid for genetic testing that is relevant to the patient's needs.

Choosing an Effective Healthcare Spokesperson: An Interactive Intervention

Advance care planning involves considering and articulating one's future medical care aspirations in advance. While ACP typically involves decision-making about specific life-sustaining therapies, choosing who will make medical decisions when the patient is incapacitated is considered by many to be the single most significant ACP action a person can take. Patients who fail to designate a healthcare spokesperson lead to confusion about who will make medical decisions, what medications are to be accepted or declined, and on what basis-which can lead to familial conflict, unnecessary and expensive medical services, and avoidable patient suffering. Surprisingly, no efforts are explicitly designed to assist people in determining the true qualities of the individual chosen as spokesperson, less require the person to state that this individual will fully represent the patient's wishes if necessary. According to three specific objectives: Aim 1. The current mixed methods study, which is based on three objectives, is intended to determine whether and how WWYC helps individuals select an appropriate spokesperson for healthcare decision making. Investigating how playing the novel game Who Would You Choose influences people's choice of a spokesperson. The investigative team will look at how WWYC influences individual players' decision making a spokesperson and if the player's decision changes as a result of game-play using qualitative methods such as focus groups and one-on-one interviews. The investigation team believes that a positive player participation with WWYC will result in successful communication with their spokesperson.

Thriving Together: Supporting Resilience in the Healthcare Workforce

Health care workers in the HCW face challenging work related situations that pose a threat to the HCW's stability and well-being. HCW education can be enhanced by hospital-based peer support services, but there are no plans and no statistics for settings outside of hospitals. The RISE hospital peer support model can be used in other health-care settings where HCW's work is exposed to distressing activities. Provide a coordinated continuum of HCW assistance services by adapting and implementing the Mental Emotional and Spiritual Help collaborative model. The RISE team is available on-call 24 hours a day to respond to calls and conduct in person or virtual first aid sessions. Starting in Year1Quarter3 in Year1Quarter3, half of the sites will be selected to receive a team of Certified RISE peer supporters. Year1Quarter2 will be published in Year1Quarter2, the step 1 survey will be conducted in Year2Quarter2, and Year3Quarter2 will be Year1Quarter2. Focus groups will be held during the intervention with 120 HCW to demonstrate quality improvement, and with 160 HCW across all levels of business to coincide with the step 1 and step 2 polls, to inquire about peer support services. Survey results will be anonymously collected through a secured online platform, according to a secure online platform. The investigators would need a total of 1056 HCW responses using a two-sided Wald Z-Test to find an effect size of 0. 3 on a measure of anxiety for independent survey samples pre- and post-intervention step, with a two-sided alpha=0. 05 and power=80%.

Do Healthcare Personnel Wearing Non-chipped, Gel Nail Polish Harbor Increased Nail Bacterial Counts in Comparison to Healthcare Personnel Without Nail Polish

The study is recruiting Geisinger workers who are 18 years old, Anesthesia resident physicians, and Certified Registered Nurse Anesthetists in the anesthesia department involved in direct patient care. Participants' baseline hand bacteria counts for each hand will be determined using the glove juice method on day zero. In other words, if a patient's left hand is randomized into the painted group, the hand will be painted, and the right hand will be unpainted, and vice versa. All participants will gather at a designated Geisinger Medical Center location on study day zero and be asked to complete a work satisfaction survey. Each of their hands' baseline bacteria counts will be determined using the glove juice method, before the subjects' hands are divided into painted and unpainted groups. Participants will be encouraged to wash their hands with soap and water prior to starting the glove juice method to obtain hand bacteria counts. Both gloves will be loaded with the premade broth for the glove juice method. The study's researchers will then stitch an elastic around both subject's wrists to ensure that the juice does not drip out of the gloves. The study's researchers would report whether or not there are chips on the nails of the hands in the nail polish business and report their findings on the subject's bacterial count sheet.
