The Role of Kynurenine Pathway and NAD + Metabolism in Myalgic Encephalomyelitis/Chronic Fatigue Syndrome.

"Myalgic encephalomyelitis/chronic fatigue syndrome is a severe, intricate, and highly debilitating long-term disease. TRYP has been attributed to the growth of certain areas of the brain responsible for behavioral functions. The kynurenine pathway is the main catabolic route for TRYP. This report explores the possibility of an elevated KP metabolism in ME/CFS. The analysis also investigates the effect of the patient's gut microbiota on TRYP availability and KP activation. We believe that NAD+-boosted NAD+ vitamins will be a promising way to combat fatigue and improve the quality of life in patients with ME/CFS. Future clinical trials will investigate the potential benefits of NAD+ supplements for reducing some of ME/CFS' clinical characteristics.

Effect of tofacitinib on pain, fatigue, health-related quality of life and work productivity in patients with active ankylosing spondylitis: results from a phase III, randomised, double-blind, placebo-controlled trial.

Tofacitinib 5 mg twice daily or placebo for 16 weeks was administered to adults with AS and with ineffective response/intolerance to non-steroidal anti-inflammatory drugs. BASDAI total spinal pain, BASDAI exhaust, ASQoL, and WPAI overall work impairment; figures increased/increased to week 48 in comparison to baseline with tofacitinib 5 mg twice daily, along with placebo in week 16; differences between baseline with tofacitinib 5 mg twice daily. By week 2, tofacitinib, and tofacitinib were both improving spinal pain and tofacitinib were noticed by week 2. Perception, physical functioning, and bodily pain domains were all higher with tofacitinib in SF-36v2 Physical Component Summary, physical function, and bodily pain domains. To week 16, which were continued to week 48, patients with AS, tofacitinib 5 mg twice a day resulted in clinically significant improvements in pain, exhaustion, HRQoL, and work quality.
