"Objective: The primary aim is to investigate the clinical and genetic phenotypes of post-infectious myelitis-chronic fatigue syndrome. " The following three study groups include 50 ME/CFS patients who recovered from COVID-19 infection, 50 non-fatigued patients with a documented history of a complete recovery from COVID-19 infection, and 50 healthy volunteers. The study has a target of finishing all research procedures on 20 of the 20 participants in each group. Up to 36 people with a community diagnosis of ME/CFS will be enrolled in focus groups to address the effects of post-exertional malaise. At the NIH Clinical Center, participants will be treated to a phenotyping tour, which will consist of a 2-5 day inpatient admission. After the phenotyping visit by a case adjudication process that used an expert medical committee and published guidelines, case status for ME/CFS participants will be determined. Participants who fulfill inclusion criteria will be able to participate in an exercise stress tour that will include a 5-10 day stay inpatient admission. "Purpose: The aim of the investigation is to determine the anti-fatigue effects of ketamine in people with chronic illness. " We recently reported that upregulation of glutamate receptors could predict people who will experience chronic fatigue one year after undergoing cancer therapy, meaning that exhaustion may have common glutamatergic markers with depression. Chronic exhaustion in a variety of medical disorders is largely triggered by similar biological mechanisms, so finding a potential anti-fatigue agent in one medical condition could be a helpful anti-fatigue therapy for other fatiguing disorders. The VAS score, which is greater than or equal to 50 mm fatigue, is regarded as the most relevant fatigue cutoff score for patients with chronic illness, but also captures the effect of a new pharmaceutical intervention for fatigue. Primary and Representative Secondary Outcomes: The primary outcome measure of the study is the change in self-reported fatigue VAS score before and three days after receiving ketamine or active comparator. In this study, a 20% decline in fatigue VAS scores three days after ketamine therapy was considered the primary indicator of effectiveness. GE's Analytic Plans: A linear mixed model with restricted maximum likelihood estimation will be used to investigate shifts in fatigue over the course of the trial, where all participants with at least a pre-dose and one post-dose test will be included. "Meditation has been widely and effectively used in RA and other rheumatic diseases, but to date, VR meditation has yet to be available in this population. " Adult outpatients with clinically diagnosed rheumatoid arthritis will be recruited from a local rheumatoid arthritis clinic and learn how to use VR in their own homes. Patients will be able to choose from many virtual outdoor settings, three styles of meditation, and three session lengths when using two handheld controllers. Patient's experiences of RA, exhaustion, as well as experience of the virtual environment will be captured in patient interviews. The findings of this feasibility study will include feasibility, acceptance, expansion, and expansion of using VR meditation for managing fatigue in outpatients with RA.
