Background and objectives Prioritizing tissue plasminogen activator (IV), data show that 4% to 7% of eligible patients with acute ischemic stroke decline II. We wanted to determine the prevalence of tPA declination in a national registry of patients with AIS in order to identify differences in declination by race/ethnicity. We compared patient demographics and identifying hospital characteristics among tPA-eligible patients who underwent and those who did not receive tPA were compared to those who refused tPA. tPA-eligible patients with AIS at 1,976 centers, 6,545 patients had tPA declination as the only cited reason for not receiving tPA. Discussion Although the overall incidence of tPA declination is low, eligible non-Hispanic Black patients are more likely to die tPA than non-Hispanic White patients, Asian Black patients are more likely to decline tPA than non-Hispanic White patients. It could be a good idea to address differences in stroke care by lowering the incidence of tPA declinations among non-Hispanic Black patients.
