Purpose Carotid artery stenting is an alternative treatment option to carotid endarterectomy. Methods We retrospectively reviewed 308 patients with carotid stenosis who underwent CAS between April 2008 and April 2020. In patients with ISR, contributing factors are also investigated. On T2-weighted imaging on black-blood magnetic resonance imaging, the ISR incidence was higher among patients with a plaque/muscle ratio of u2264 2. 30. On BB MRI, a plaque/muscle ratio of u2264 2. 30 was found to be an independent risk factor for ISR. On MRI T2WI, there was also a comparison of the prevalence of ISR and the plaque/muscle ratio ratio. The cutoff value for detecting ISR from the Plaque/Muscle ratio was 1. 310 [area under the curve = 0. 9 percent, specificity = 85. 2%], according to T2WI imaging of BB MRI. T2WI in pre-intervention MRI plaque imaging may be a useful predictor of post-intervention ISR.
