Mycotic aneurysms of the intrapetrous carotid artery are a rare complication of adjacent center ear infections that take place by direct invasion of arterial adventitia. We report the first case of a Pseudomonas aeruginosa mycotic intrapetrous carotid aneurysm arising from mastoiditis, confirmed with center ear cultures, providing with state-of-the-art bacteraemia and otorrhagia in a diabetic man. We provide our experience to highlight the threats of invasive P aeruginosa middle ear infections in patients with diabetic issues to various other specialists and to encourage earlier, extra aggressive treatment.
