Abstract Background The IVC extension's methods for renal tumor resection are based on individual unit experience. a Methods Over six years there were nine patients with renal cell carcinoma invading the IVC, underwent surgery. CPB was used in 8 and hypothermia and circulatory arrest in all patients with level IV disease. Mortality: 2 patients Conclusions: The IVC from an adherent tumor must be cleared, so extensive level IV disease presents a surgical challenge. For level III and brief periods of Total Circulatory Arrest for level IV disease, we recommend CPB.
