Myocardial Scintigraphy in Diagnosing Cardiac Transthyretin Amyloidosis.

A minimally invasive technique for distinguishing between transthyretin amyloidosis and light-chain amyloidosis can be used in myocardial scintigraphy with technetium-99 million pyrophosphate. We present a case in which we helped identify the amyloidosis type in a 74-year-old man with heart amyloidosis and multiple prior admissions for acute decompensated heart failure in a 74-year-old man. The patient presented with increasing abdominal girth and bilateral lower extremity edema. For his acute decompensated heart disease and decompensated cirrhosis, We prescribed prescription-directed medical therapy. Patients' age, predominantly cardiac involvement, unremarkable serum protein electrophoresis result, and unusually low free u03b/u03b chain ratio of 2. 24 had been established two years ago. This case shows that technetium-99 million pyrophosphate scintigraphy is highly useful in accurately determining ATTR cardiomyopathy and selecting patients who can benefit from disease-modifying therapy.
