Neoadjuvant PD-1 blockade may be safe in patients with acute, resectable oral-cavity, head-and-neck cancer. We investigated longitudinal tumor and blood samples in a cohort of 12 patients demonstrating 33% responsiveness in an investigation of correlations of neoadjuvant nivolumab therapy and post-surgical recurrences. T-cell receptor repertoire diversification in peripheral blood and intratumoral expansion of preexisting T-cell clones is triggered by chemotherapy, immunologically, and tumor formation in intravascular T-cell clones. In pretreatment blood, a high percentage of regulatory to Th17 T cells suggests genetic resistance, low cytolytic T-cell signature in pretreatment tumor, and poor T-cell receptor repertoire in pretreatment blood.

Differential neoadjuvant chemotherapy response in urothelial carcinoma molecular subtypes

There are no biomarkers in clinical use that can show patients that are sensitive or insensitive to neoadjuvant chemotherapy. This paper explores how molecular subtypes influence patient response and longevity in 149 patients receiving preoperative cis-platin-based chemotherapy by tumor classification using transcriptomic profiling and a 13-marker immunostaining panel. In addition, we investigated to see whether gene expression signatures could predict chemotherapy response beyond molecular subtypes. Overall plan: Retrospective cohort study aimed at determining the connection between molecular subtypes and neoadjuvant chemotherapy response in muscle-invasive bladder cancer.
